Job Title: Social Worker (MSW)

Job Summary

Acts as an advocate to the residents and families of the MJH long term care nursing home area by delivering a broad spectrum of social work services in a qualitative, empathetic, and caring manner.

Essential Job Functions

•       Supports, counsels, and acts as an advocate for residents and family.
•      Completes all necessary and required social work documentation to include psychosocial assessments, treatment plans, quarterly evaluations, MDS, supplemental notes, discharge plans etc
•      Coordinate and facilitate patients discharges by ordering all services needed post discharge from facility
•      Interfaces with interdisciplinary staff members to share information on the resident
•      Assists with discharge planning in a rehab setting
•      Facilitates support groups
•      Attends meetings as needed

Job Requirements

Education

•      Master’s degree in clinical social work required

Experience

•      Three years of related experience

Licenses/Certifications

•      N/A

Abilities Required

•      Excellent assessment and counseling skills
•      Ability to work in a fast-paced social work position that involves discharge planning from a rehab setting
•      Must be computer literate
•      Excellent communication and interpersonal skills
•      Knowledge of geriatrics/gerontology preferred
•      Ability to work closely and well with other disciplines
•      Thorough knowledge of psychosocial dynamics of individual & family

Functional Demands

Environment /Work Conditions:

•      Normal working condition with adequate lighting and ventilation

Infectious Material Exposure:

•      No routine exposure to blood/body fluids/tissues but may occasionally require performing unplanned activities which may involve exposure to blood/body fluids/tissues
